What services does Revive Medical Care offer?

We provide medical weight loss (GLP-1/semaglutide), testosterone replacement therapy (TRT), IV hydration therapy, peptide therapy, erectile dysfunction treatment, direct primary care memberships, performance medicine, onsite lab testing, and wellness care. Available in-office in Goodyear, AZ or via telehealth across Arizona, Colorado, New Mexico, Florida, Nevada, Utah, Texas, and Washington.

Do you accept insurance?

Yes, we work with most major insurance providers for many services including weight loss programs, IV hydration, lab testing, and primary care. Direct Primary Care memberships are insurance-independent with flat monthly pricing.

Do you offer Semaglutide or GLP-1 injections?

Yes. We prescribe FDA-approved GLP-1 medications including semaglutide as part of our medical weight loss program. Treatment begins with a consultation to assess medical history, goals, and eligibility.

How quickly can I get an appointment?

We offer same-week appointments for new patients in most cases. Book online through our Elation patient portal or call (480) 389-7079.

What is TRT and who is it for?

Testosterone Replacement Therapy helps men experiencing low testosterone regain energy, strength, mental clarity, and libido with customized protocols and regular lab monitoring.

Can I get care via telehealth?

Yes. Most services including weight loss, TRT, and ED consultations are available via telehealth. We currently serve patients in Arizona, Colorado, New Mexico, Florida, Nevada, Utah, Texas, and Washington.

What is Direct Primary Care?

DPC is a membership-based model with direct provider access, discounted labs and medications, and longer appointments for a flat monthly fee starting at $98/month. No insurance middlemen and no surprise bills.
