Revive Medical Care Terms & Conditions – New Patient Referral Discount Program
Program Overview
Revive Medical Care (“Practice,” “we,” “us,” or “our”) offers a 20% new patient referral discount program for eligible existing patients, subject to the terms and conditions below.
This program is promotional in nature and may be modified or discontinued at any time in our sole discretion.
Eligibility Requirements
The referrer must be an existing patient of Revive Medical Care in good standing at the time the referral is made, and the patient’s account must be current on all payments and not in collections.
The referred individual must be a new patient to Revive Medical Care who has never before received services at any Revive Medical Care location.
To qualify, the new patient must: Schedule and complete an initial visit within 30 days of the referral date, and Identify the referring existing patient at the time of booking or check-in, or use any referral code or process designated by Revive Medical Care.
Discount Details
Upon verification of an eligible referral, the existing patient will receive a one-time 20% discount applied to services at the existing patient’s next visit only.
The discount: Applies to eligible, self-pay services rendered at that next visit, as defined by Revive Medical Care. Does not apply to past visits, unpaid balances, membership fees, products, supplements, gift cards, laboratory fees, imaging, or services billed to or reimbursed by insurance. Cannot be combined with other promotions, coupons, package pricing, membership discounts, or special offers, unless expressly stated in writing by Revive Medical Care.
If the existing patient does not use the discount at the next visit, the discount is void and will not roll over to subsequent visits.
Program Limitations
A maximum number of referral discounts per existing patient may be imposed and may change at any time without notice.
Referrals must comply with all applicable federal and state laws, including any regulations governing patient referrals and discounts in healthcare.
Revive Medical Care reserves the right to verify eligibility, refuse to honor any referral it reasonably believes was obtained or used fraudulently, and to correct any errors related to the administration of this program.
Non‑Transferability and No Cash Value
Referral discounts have no cash value and may not be sold, assigned, transferred, or exchanged for cash, credit, or any other benefit.
Only the existing patient identified as the referrer may receive and use the applicable referral discount.
No Impact on Clinical Judgment
Participation in this referral discount program does not influence or replace independent medical judgment.
All treatment decisions are based solely on the clinical needs and best interests of the patient, and participation in the program does not guarantee the availability of any particular service or provider.
Privacy and Confidentiality
Any information collected in connection with the referral program will be handled in accordance with our Notice of Privacy Practices and applicable privacy laws.
Revive Medical Care will not disclose the identity of the referrer or referred patient except as permitted or required by law or as necessary to administer this program.
Program Changes and Termination
Revive Medical Care may amend, suspend, or terminate this referral discount program, or these Terms & Conditions, at any time without prior notice.
Any changes will be effective upon posting updated Terms & Conditions on this webpage, and continued participation in the program after such posting constitutes acceptance of the updated terms.
Acceptance of Terms
By participating in the Revive Medical Care new patient referral discount program, both existing and new patients acknowledge that they have read, understood, and agree to be bound by these Terms & Conditions.
